This form is for Recharge merchants that are ready to convert from their existing Recharge app on Shopify to the Shopify Checkout-integrated version of the Recharge app. This form is intended for one live store only, if you have multiple stores an additional form will need to be created for each store.

Once you submit the form we will be in touch in the next few weeks to let you know roughly where you'll be in the queue and when you can expect to begin the conversion process.

We will not make any changes to your setup without your knowledge and approval.
